HCP ClientWhat can Home Care Packages funds be use for?
CARE MANAGEMENT
Regular assessments to ensure you receive the care you need. This includes reviewing your needs and preferences, updating your care plan, coordinating with other supports, ensuring culturally safe care, and addressing any risks to your safety, health, and well-being.
PERSONAL SERVICES
Assistance with personal activities such a bathing, showering, toileting, dressing and undressing, mobility and communication.
MOBILITY AND DEXTERITY
Providing crutches, quadruped walkers, walking frames, walking sticks, mechanical devices for lifting, bed rails, slide sheets, sheepskins, tri-pillows, pressure-relieving mattresses and assistance using these aids.
NURSING, ALLIED HEALTH AND THERAPY SERVICES
For example, this may include speech therapy, podiatry, occupational therapy or physiotherapy services and other clinical services.
TRANSPORT AND PERSONAL ASSISTANCE
Assistance with shopping, visiting health practitioners and attending social activities.
MEAL PREPARATION
Includes meal preparation, special diet requirements, assistance with eating utensils and eating aids, direct feeding help, and providing enteral feeding formula and equipment.
CONTINENCE MANAGEMENT
Assistance in using continence aids and appliances such as disposable pads and absorbent aids, commode chairs, bedpans and urinals, catheter and urinary drainage appliances, and enemas.
HOME MAINTENANCE & DOMESTIC ASSITANCE
Assistance with bandages, dressings and skin emollients.
RESPITE CARE
one-on-one support and companionship, to give carers and family members a much-needed break.
You are eligible for a home care package if you are:
- A person over 65 years old who needs coordinated services to help you to stay in your home.
- A person over 50 years old with a disability, dementia or other special care needs that are not met through other specialist services.

Go to www.myagedcare.gov.au/referral or contact My Aged Care at 1800 200 422 to MAKE AN APPLICATION for a Home Care Package.
Schedule a visit
An advisor from MyAged Care will personally contact you to schedule an appointment. During this appointment, they will conduct a comprehensive assessment of your situation. This will involve discussing your current needs and any health concerns you might have
Client assessment
On the scheduled date, a qualified MyAged Care advisor will visit you at your home to conduct the assessment. This visit allows the advisor to understand your environment and personal needs more accurately.
*Please note that there may be a wait time of 3 to 12 months to receive the package.*

Once you choose to receive services from United, one of our dedicated Case Managers will reach out to you to coordinate the necessary services designed to enhance your quality of life and promote your independence. Our Case Manager will work closely with you to understand your unique needs and preferences, ensuring that every service we provide is tailored to support your lifestyle and personal requirements. At United our goal is to enable you to maintain control over your life choices and flourish in your independence.
Start Services
After your consultation with the Case Manager and the subsequent preparation of your care plan, a representative from our Rostering team will contact you to schedule the dates and times for your services. This will ensure that you are able to fully benefit from your package.
